I understand that limited assistance will be available in-person. I agree to review the training material prior to my appointment to ensure I am prepared. If you would like to request assistance during an appointment, please reach out to Laura at [email protected] prior to your appointment.

I will review the safety guidelines shared with me by email before using the machine. I hereby release and hold harmless the New Canaan Library, Inc., its officers, employees, volunteers, committees and boards, from and against all liability, and I and any parent or guardian signing for me below hereby agree to indemnify them against any loss, damages, claims, costs (including attorneys' fees) or actions of any type, resulting from bodily injury, property damage, or any other loss that may result from my use of the 3D printer.
